We Drive Health values providing clean and safe home-based primary care services to patients and their families. We follow CDC guidelines closely to optimize the experience we can provide. During our visits:

  • Masks for all staff and patients will be worn & reinforced
  • Disposal equipment will be utilized as much as possible
  • Patient appointments are staggered to ensure staff sanitize all equipment rigorously
  • Allowing for optional telehealth services to maximize the comfort level of every patient
We will determine this based on your needs and as per your insurance coverage. One of our providers will see you at least twice a year for primary care services.
We will come to your residence, review your medical record, discuss your health and progress, examine you and order any appropriate testing and imaging.
While we do not directly provide these additional services, we coordinate services with home health nurses, occupational and physical therapists who play a critical role in your functional development.
We will escalate to a higher level of care and facilitate transport to the emergency department, or if deemed appropriate, will transition you to hospice service.

WeDriveHealth offers a range of primary care, preventive care, and chronic disease management services, as well as various wellness services such as nutrition counseling and stress management. We also offer alternative and complementary therapies, such as acupuncture, chiropractic care, and naturopathic medicine.

WeDriveHealth accepts most major insurance plans, including Medicare and Medicaid. Please check with our front desk to ensure that we accept your specific insurance plan.

Yes, WeDriveHealth is a patient-centered medical home, which means that we are dedicated to providing coordinated and comprehensive care that is centered around your unique needs and goals.
You can schedule an appointment with WeDriveHealth by calling our front desk or visiting our website to schedule an appointment online.
If you need to cancel or reschedule an appointment, please call our front desk as soon as possible. We ask that you give at least 24 hours notice when canceling an appointment to allow other patients to schedule in that slot.
Yes, WeDriveHealth offers telemedicine services to allow patients to have virtual consultations with our healthcare providers. Please contact us for more information on how to schedule a telemedicine appointment.
Patients can request their medical records by contacting our front desk and filling out the necessary paperwork.
Patients can request a referral by scheduling an appointment with their primary care provider, who will then provide a referral if necessary.
